 PARENTS – PLEASE NOTE:  WHEN MAKING ANY CHANGES TO YOUR CHILD’S TRANSPORTATION NEEDS WHETHER IT IS FOR ONE DAY, ONE WEEK, OR PERMANENT PLEASE LOG ON TO THE FCPS WEBSITE:  http://www.cityschools.com/parents/ and follow the directions below.

 SELECT LUES FROM THE SCHOOL DROPDOWN BOX, CLICK ON TRANSPORTATION CHANGES TAB AT THE TOP OF THE PAGE.  AN AUTO-FILL FORM WILL OPEN FOR YOU TO FILL IN THE INDICATED FIELDS.  PELASE COMPLETE ALL FIELDS BEFORE SUBMITTING YOUR CHANGES/REQUESTS.  PLEASE BE SPECIFIC WHEN COMPLETING THIS FORM.  STUDENTS NAME(S), STREET ADDRESS, AND YOUR CONTACT INFORMATION IS REQUIRED.  AN E-MAIL WILL BE SENT TO THE TRANSPORTATION SECRETARY, AND YOU WILL RECEIVE  A RESPONSE THAT IT WAS RECEIVED AND PROCESSED
